A NASA Apollo 13 mission photograph showing rocket liftoff from Kennedy Space Center on April 11, 1970, signed and inscribed in black felt tip pen by American astronaut Fred W. Haise, Jr. (born 1933), as "Fred Haise / Apollo 13 LMP" in the upper right corner. The NASA image, (MSCL-59), is in brilliant glossy color, with printed information found verso. Expected surface wear including minor scuffs and gentle wrinkles, as well as isolated pin holes and old tape residue found along the top edge. Several deep wrinkles affecting the lower right and upper left corners, else very good. 8" x 10."

Fred W. Haise served as the Lunar Module Pilot of the ill-fated Apollo 13 space mission. The 3-person crew was comprised of Haise, Commander Jim A. Lovell, Jr. (born 1928), and Command Module Pilot Jack L. Swigert, Jr. (1931-1982). Apollo 13 was earmarked to make the third ever moon landing after its April 11th liftoff and during its mid-April 1970 space flight. Yet the mission was aborted after an oxygen tank in the spacecraft's service module exploded on April 14th, forcing the crew to transfer to the lunar module, which acted as a lifeboat. The crew built ersatz equipment using existing materials to perform essential functions, like carbon dioxide filtering, and severely rationed supplies. Resilience, ingenuity, and outstanding communication between ground support and space enabled the astronauts to safely return to Earth on April 17th. Apollo 13 was, in the words of Commander Lovell, a most "successful failure."

After Apollo 13, Haise commanded the Space Shuttle "Enterprise" during a 9-month-long period of approach and landing captive and free flight tests conducted between January and August 1977. These trials enabled NASA to gauge how the space plane would perform under a variety of conditions.
